Pest invoices get muddy when a one-time kitchen visit is written like a year of service. This sample is the first trip only: the visit, six stations you can count, and gel in the kitchen.
One visit for ants and occasional invaders. Stations and gel are listed. Pay when the tech leaves.
A quarterly plan. That would name the next months and a repeating amount. Do not imply it here.

What the person who was not home still needs to see
- Service address. It may differ from the billing name.
- What you came for, in plain words. Ants, not a product code alone.
- How many stations, and that they are outside.
- What you did inside, separately.
- Whether a return visit is included, and for how long. Fourteen days in this story.
- A license number in your business details if your state uses one.
Due at the end of the visit
Dana Feldman pays the same day. There is no net-30 float on a kitchen that was treated this morning. A monthly office clean is a different rhythm, covered on the cleaning template. A yard visit with mulch is the landscaping template.
One visit versus a plan
How is a first visit different from a quarterly plan on the invoice?
The first visit is a finished piece of work with a count of what you installed. A plan is a schedule. This sample says, in the client block and the notes, that it is not a quarterly plan. If you sold the plan, bill the plan, and name the months.
Why count bait stations?
Six stations at an example $15 means the customer can walk outside and agree with the number. 'Exterior service' with no count is harder to remember next quarter.
Is the follow-up a second invoice?
Not in this example. A return within 14 days is included and is not priced again. If you charge for callbacks, say that before the first visit, then put it on the bill when it happens.
Where does a license number go?
With your business details, under your company name. Harbor Pest's download does not invent a real license. If your state issues applicator numbers, add yours there. Check your state's rules rather than guessing from this page.
Should product names be on the customer invoice?
A short name helps if someone asks what was used in the kitchen. You do not need a full label essay. Write what you are willing to stand behind, and keep the longer record in your own notes.
